Dalia Amrom, MD

Bio: Dalia was born in Israel and grew up in Toronto, Canada, were she completed her undergraduate studies in Kinesiology and Health Sciences. She worked as a Registered Kinesiologist, and then moved to Europe to study medicine in the beautiful city of Krakow, Poland. After medical school, Dalia flew back across the pond to complete her residency in Internal Medicine at Lankenau Medical Center in Philadelphia. Serendipitously, she met her partner in Philadelphia, and is excited to call this city her new home! Dalia is looking forward to working with a wonderful interdisciplinary team as she completes her Hospice and Palliative Care Fellowship this year. 

Clinical Interest:  Dalia is passionate about working with underserved populations living with serious illnesses, expanding access to healthcare among marginalized communities, enhancing medical education, and providing continuity of care in the outpatient palliative care setting.

Personal Interest:  Outside of work, Dalia enjoys spending time outdoors, hiking, running and biking. She loves traveling and learning new cultures, and has visited nearly 25 countries with more on the horizon! She also enjoys refurbishing furniture, playing tennis, and caring for her large plant collection.

Colleen Graham, DO

Bio: I was born and raised in South Jersey. I then went to college at Wagner College in Staten Island where I majored Chemistry and minored in Biology while completing pre-med courses. I then moved back to the Philadelphia area where I attended medical school at The Philadelphia College of Osteopathic Medicine. I then matched at Jefferson Health NJ for Internal Medicine residency and then in Hospice and Palliative Care at Jefferson University Hospital for fellowship. Along the way, I got married and had our sweet little baby girl. 

Clinical Interests: My professional interests include inpatient hospice, inpatient palliative medicine, and academic medicine.

Personal Interests: In my spare time, I enjoy spending quality time with my daughter and husband, going to Cape May, watching football (Go Birds!), and baking.

Jaclyn Nguyen, MD

Bio: I am originally from Texas and have lived in all three of the major metroplexes that make up the state-wide triangle. I was born and raised outside of Houston, TX before moving to Austin, TX for college (University of Texas at Austin) and eventually matriculated to Dallas, TX for medical school (University of Texas Southwestern). Afterwards, I crossed the Rocky Mountains to complete my internal medicine residency in Billings, MT surrounded by mountain scenery. Now I find myself in Philadelphia, PA after trekking the Appalachians to complete my hospice and palliative care fellowship at Jefferson.

Clinical Interests: My professional interests include embedded ICU palliative care,  inpatient hospice, and longitudinal care of non-oncology palliative care patients.

Personal Interests: I enjoy outdoorsy activities with my partner such as hiking, biking, and snowboarding! I also enjoy solidly indoor activities with our cat, such as stationery/sticker collecting and baking.
