Thomas Jefferson University Hospital
This 700+ bed acute care hospital provides quaternary care for Philadelphia and the surrounding region. The majority of clinical rotations take place at this location. Fellows working here have the opportunity to care for patients in multiple settings, including outpatient clinics, general medicine and subspecialty wards, and medical and cardiac intensive care units. Given its location in Center City Philadelphia, Thomas Jefferson University Hospital serves patients from a wide range of socioeconomic and ethnic backgrounds, which provides our fellows with a diverse educational experience.
Thomas Jefferson University Hospital will serve as the main teaching site for the HPM fellowship. Fellows will become core members of the inpatient palliative care team and will provide inpatient consultation for a wide variety of patients treating a variety of conditions. Fellows will also have the opportunity to do elective rotations on site with other programs based on the fellow’s interest.
Vitas Hospice
Located at one of our affiliate hospital sites at Jefferson Methodist Hospital, this site will be another hospice site where fellows will care for patients. The Vitas Hospice unit at Methodist has 16 beds and cares for many of the patients seen in Center City, providing outstanding continuity experiences for our fellows.
The Hill at Whitemarsh & Watermark
This continuing care retirement community (CCRC) is located just minutes outside of Philadelphia and is a community for older adults of all stages of the geriatrics spectrum from fully independent to nursing home bound. Here fellows will work closely with Board Certified Geriatricians on providing Palliative Care Consultation in this unique population.
Outpatient Palliative Care at Jefferson Geriatrics
Started in 2018, Palliative Care at Jefferson Geriatrics serves to provide HPM consultation for elderly patients. We have also begun seeing patients from both the cardiology and pulmonary practices here. Providers in this clinic also provide medical marijuana certification.
ALS Multidisciplinary Clinic
During Fellowship, Fellows will have the opportunity to see patients with ALS as part of a multispecialty team including PMR, Pulmonary and Neurology. This provides a unique experience to learn how collaborative palliative care can help improve the lives of those facing serious illness.
The Neu Center for Supportive Care
Newly created in 2017, the Neu Center for Supportive Care will serve as the continuity clinic for fellows during their fellowship under the supervision of HPM faculty. While Jefferson’s Palliative Care team had previously run a well-established outpatient palliative care clinic, the Neu center will provide an expansion of outpatient services for patients who are able to come in for visits but also for those who cannot. Fellows will have the opportunity to do home, telehealth, nursing home and subacute rehab palliative care type visits providing continuity across a variety of locations for patients throughout the health system.
Pediatric Palliative Care at A.I. du Pont Hospital for Children
The Nemours/Alfred I. duPont Hospital for Children is a multi-specialty, tertiary care teaching institution located on a 300-acre estate in the scenic Brandywine Valley in Wilmington, Del. This large pediatrics hospital is home to a well-developed Pediatric Palliative Medicine program with board certified HPM faculty. Fellows will spend 2-4 weeks rotating at DuPont caring for chronically and critically ill pediatric patients and their families.
