What is Care Management?

The Student Counseling Center (SCC) offers clinical care management services to provide students with information about on and off-campus resources that can support their wellness and success in school. If it is determined that a student’s needs fall outside of brief treatment in our center, students can meet with a care manager to explore community treatment options. The primary goals of care management are to assist students with locating relevant resources, provide linkage options for long-term therapy and psychiatric care, and provide therapeutic support while students await linkage from SCC brief treatment to long-term care in the community. 

Welltrack Connect

All students enrolled at Thomas Jefferson University are eligible for mental health services through the East Falls or Center City Student Counseling Center. These services are provided free of charge to students and fall within a brief treatment model. If students determine they would like to establish long-term mental health care in the community, Welltrack Connect provides a straightforward path for locating care. 

Welltrack Connect is a free website designed to help students search for mental health services in the community. The site enables students to search for providers based on desired type of treatment, preferred location, shared availability, in-network insurance eligibility, provider identities, and areas of expertise. Additionally, students can message community providers directly through the Welltrack Connect portal to ask questions or schedule an initial appointment. The Student Counseling Center also uses Welltrack Connect to collaborate with local providers to establish working relationships in settings where students are regularly referred.

Welltrack Connect also houses a number of other resources that could be of use outside of mental health care. Community Resource Guide

This community resource guide, compiled by the Office of Academic & Career Success, includes a list of Philadelphia-area organizations and resources that are equipped to address concerns related to food insecurity, housing concerns, childcare, and access to government benefits. This resource guide is intended to provide an initial direction to community support and should not be considered a comprehensive list of all local resources available. This guide was updated in September 2022.
