Physical Exam

Student Health Medical Record Requirements

Students are required to submit a completed Student Health Record Form (PDF), which includes the student's medical history and a recent physical exam and documentation of required immunizations completed and signed by a health care professional.

Transfer students must also submit these form.

The University reserves the right to deny housing or to withhold registration for any student who does not meet the health form requirements.

Where can I get a physical exam?

You may obtain a physical exam by:

• An outside healthcare professional in private practice
• An outside convenience or urgent care clinic
• A local public health department
