  • Innovative educational model from the University of New Mexico.
  • Focuses on complex conditions requiring a team-based approach to care.
  • Builds primary care workforce capacity and learning communities via Zoom.
  • Improves access to experts to enhance clinical practice and healthcare delivery.
  • Provides frontline clinical teams with knowledge and support needed to manage underserved patients in their own communities.
  • Is offered at no cost to participants.

Continuing Education Credit

 Continuing education credits can be earned for each session attended.

Physicians This educational activity is designated for a maximum of 1.00 AMA PRA Category 1 Credit™ per session. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Nurses This activity awards 1.00 nursing contact hour per session. 
Psychologists, Social Workers, & Behavioral Health Practitioners This activity awards 1.00 CE credits.
Other Allied Health Professions This accredited continuing education activity has been approved for CE Credit.
