Student Health Medical Record Requirements
In order to assure the health and safety of our students, faculty, staff and visitors, Thomas Jefferson University Health Forms compliance is REQUIRED. Please go to the Student Health Services Patient Portal (https://jefferson.medicatconnect.com) to upload required forms, enter immunization dates, and complete required forms. You will need to log in using your Thomas Jefferson University Email formatted as [email protected] (use your actual campus key email). If you are prompted to select a university from the drop down, please make sure to select Thomas Jefferson University.

Transfer students must also meet the above requirements.
The University reserves the right to deny housing or to withhold registration for any student who does not meet the health form requirements.
