- Addiction, Serious Mental Illness, & Health Equity The mission of this group is to end health disparities through rigorous stakeholder-driven methodology for people with serious mental illnesses and/or people who use substances
- Cancer Care Across the Continuum - The cancer care continuum team within the Department of Family and Community Medicine focuses on patient, provider and community-centered interventions to improve cancer prevention, screening and care across the continuum, particularly in vulnerable populations.
- Chronic Care Management - To develop and evaluate innovative chronic illness management, primary care education and training, and primary care practice transformation initiatives.
- Geriatric Medicine & Palliative Care - The mission of the Division of Geriatric Medicine and Palliative Care is to optimize the health and quality of life of older persons and those with serious illness by partnering with patients, families and communities.
- Integrated Community Health - To achieve equity in health care access and outcomes for underserved population through community based collaborative programs through Thomas Jefferson University Hospital’s Center for Urban Health, Jefferson’s community health clinics, and in partnership with numerous community-based organizations.
- Medical Education & Training Research The Medical Education and Training team develop and evaluates education and training on quality improvement, population health, leadership, and new models of primary care delivery across the educational continuum.
- Primary Care Practice Transformation The Primary Care Practice Transformation group develops and evaluates innovative primary care practice transformation initiatives.
- Primary Care Clinical Trials - (PCCT) performs a wide variety of clinical research studies under the auspices of the Department of Family and Community Medicine at Thomas Jefferson University. Our experienced research staff includes full-time Faculty, Clinical Fellows, a Clinical Research Coordinator, Medical Techs and other administrative professionals. We have over 25 years of experience in Pharmaceutical industry–supported clinical trials.
- Refugee & Immigrant Health - The mission is to achieve equity in healthcare access and health outcomes for refugees resettled in the United States.
